Families with children and youth with special health care needs (CYSHCN) have additional and evolving needs for staying safe in emergencies and often also experience major hardships across the social determinants of health (SDOH), even when connected to a medical home. We found that a virtual home preparedness intervention (VHPI) boosted preparedness metrics in a diverse, statewide sample (n = 170) and enabled identification of 1,072 unmet needs that were addressed through the medical home and community partners. By coupling preparedness achievement with identifying and ameliorating SDOH hardships, our findings suggest that medical home providers may effectively support both these crucial areas during preventive care visits for CYSHCN.
